Job Description

Our Nurses work in collaborative environments with strong interdisciplinary teams, focused on a patient centered care model in a richly diverse work space. Under the direction and general supervision of the Practice Manager, the registered nurse utilizes evidence based practices in accordance with the State Nurse Practice Act, policies and procedures of the hospital and as directed by the medical staff.

This position will be involved in the care of several specialties, which currently include orthopedics, transplant services, gastrointestinal, trauma, neurology, cardiology, and infectious disease. We work as a team in this fast paced environment and care for our local population. Our ideal candidate can easily talk to patients, help answer questions and address concerns.

• Provides office-based nursing care in collaboration with provider
  
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care through appropriate nursing evaluation and intervention
  
• Communicates with provider regarding patient needs, nursing assessments, and recommendations
  
• Takes direction and initiates actions (cross/additional training) that will allow seamless patient care
  
• Collects and documents current health status and medical history and assists in scheduling patient office visits
  
• Reviews patient records, including preoperative records, to ensure completeness and reviews outstanding issues with provider
  
• Demonstrates independent nursing actions based on assessment and problem identification
  
• Under direction of provider, calls in prescription medications to pharmacies and observes and documents patient's response to pertinent medications
  
• Documents accurate and ongoing assessment of patient's needs, problems, capabilities, limitations, progress toward goals, and response to care
  
• Triages telephone calls and displays ability to initiate appropriate nursing intervention in prompt, precise, and professional manner
  
• Assists patients and family members or other clients with concern and empathy; respects confidentiality and privacy
  
  Qualifications Education Requirements:
  
  --- BSN (preferred)
  
  Minimum Requirements:
  
  --- Current RN license in state of Connecticut
  
  --- Ability to manage multiple priorities in a high volume environment.
  
  --- Self-directed in the management of patient care.
  
  --- Highly effective verbal and written communication skills.
  
  --- Independent problem solving skills
