Registered DietitianLord Chamberlain is actively seeking A
Registered Dietitian to join our Dynamic team of professionals!We
serve patients in long term, short-term or sub-acute
(rehabilitation) settings.License/Certification: Registered
Dietitian License (Required)Duties and
ResponsibilitiesAdministrative Functions Accurately and
appropriately determines the nutritional status of residents
through completion of nutritional assessments based on facility
policies and in accordance with time frames outlined in facility
requirements as per facility policies. Appropriately determines the
nutritional and hydration risk level of residents. Assesses the
adequacy of calorie, macronutrient, and fluid intake within
facility guidelines. Determines nutrient requirements based on
department criteria. Report all accidents and incidents you observe
on the shift that they occur.Actively collects, reviews and
assesses anthropometric data, biochemical parameters (eg, sodium,
potassium, albumin, blood urea nitrogen), past and current medical
and social history and food intake and/or family member, and health
care team to assess nutrition and nutrition education needs of
patients. Identifies need for nutrition supplementation for
residents. Reviews, collects, inputs in computer, and assesses
monthly and/or weekly weight data. Includes religious, cultural,
and ethic factors, as well as residents wishes when completing
nutritional assessments. Develops meaningful and individualized
nutritional interventions based on the assessed needs of the
residents and documents in the care plans. Provides timely
follow-up monitoring and evaluation of residents identified to be a
nutrition risk. Identifies appropriate tube feeding formula product
within the facility formulary to meet the resident requirements.
Develops appropriate interventions to maintain or improve the
nutritional status of the resident. Continuously monitors and
evaluates the nutrition care and outcomes of
residents.Qualifications of a Registered Dietitian: Must have
effective communication skills, both verbal and written Must be
creative and have good observations and analytical skillsEducation
requirements: Minimal requirements are a bachelors Degree in
nutrition or related degree. One year of appropriate clinical
