Job Description:
Jersey Shore Post Acute, a leader in post-acute care and
rehabilitation services, is seeking a compassionate and dedicated
Licensed Practical Nurse (LPN) to join our team. This role involves
working closely with patients recovering from various medical
conditions, providing them with high-quality nursing care tailored
to individual health needs. As part of our professional nursing
team, you will play a crucial role in our patient-centered care
approach, administering prescribed medications, assisting in
rehabilitation exercises, and monitoring patient progress. You will
be expected to adhere strictly to professional nursing standards,
contributing significantly to our mission of enhancing patient
wellness and facilitating quicker recovery. Administer nursing care
to patients as per the treatment plan Monitor and record patients'
condition and document necessary reports Assist in the performance
of diagnostic tests and analyze results Administer medications as
prescribed and monitor responses Help with patient mobility and
physical therapy exercises Educate patients and their families on
disease prevention and health maintenance Collaborate with the
healthcare team to optimize patient care and recovery Strong
communication and interpersonal skills Proficient in maintaining
accurate patient records Ability to respond effectively to the
needs of patients Capable of managing multiple tasks in a dynamic
environment Empathetic and compassionate approach to patient care
Medical Insurance Dental Insurance Vision Insurance Current
Licensed Practical Nurse (LPN) license in the state of employment
CPR certified
